REGISTRATION FORM 2012-2013
Registration fee is $15.00 per student, or $25.00 per family. Class times and sizes are limited, so please turn in registration by Friday, June 15th. Payment must accompany registration. All registration fees are non-refundable. Please make checks payable to Ali Wierman.

*Beginner Pointe is for students twelve and up who have had a minimum of three years of ballet. The Pointe class is for all dancers who have previously taken this class. Pointe class is separate from ballet; any student who registers for this must also register for ballet.
